Title: Do sanctions work?

I'm an economics dummy and am wondering if countries that have been sanctioned (such as N.Korea and Iran) really do get hurt, and if so by how much and in what way?
I mean, are there suddenly food shortages or what?
And no doubt the leaders of those countries don't suffer at all and simply carry on living in their palaces and mansions with well-stocked pantries?
